What do I bring to my Office Visit?

 

• Insurance card

• Photo ID

• Medication bottles

• Labs or medical records

 

Medication refills have to be requested TEN business days BEFORE medication is depleted. Refills will be reviewed by the Doctor for approval or denial within TWO business days.

 

We ask our patients to bring medication bottles at each visit.

The kidneys are responsible for filtering many drugs and it is imperative that we review them periodically.

 

If there is any change in insurance, we ask you to bring in the appropriate cards. All payments including copays are due at date of service.

 

If you do not have insurance, please discuss the payment plan with our staff prior to your visit.

 

We accept credit cards (includes: credit card processing fees) and cash as forms of payment

 

If you are unable to keep your appointment please contact our office 24 hours PRIOR to the appointment, or a $25.00 fee will be applied to your account.

 

If you require any copies of medical records or forms to be filled, processing can take 5-10 business days and may incur additional fees.
